What problems always cause a drug test to be cancelled?
A fatal flaw cannot be corrected and will always cause a DOT drug test to be cancelled. Here is a list of fatal flaws that may occur during a DOT drug test:
- The collector did not print or sign their name in Step 4 of the CCF
- The specimen ID numbers on the specimen bottles and the CCF do not match
- The specimen bottle seal is broken or shows signs of tampering
- There is an insufficient amount of urine in the primary specimen bottle (30 mL in bottle A) and the specimens cannot be re-designated
- A CCF arrives to the laboratory without a specimen
- A specimen arrives to the laboratory without a CCF
- If two separate collections were performed using only one CCF
